???? What You’ll Do
As a Home Health RN, you will serve as the clinical leader in the home setting, responsible for:
- ???? Performing comprehensive patient assessments (physical, psychosocial, functional, and environmental)
- ???? Developing, implementing, and updating personalized plans of care
- ???? Providing skilled nursing care for patients with complex and chronic conditions
- ????️????️???? Educating patients and families on self-care, medications, nutrition, and disease management
- ???? Evaluating patient progress and adjusting care plans as needed
- ???? Communicating changes in patient status to physicians and interdisciplinary team members
- ???? Ensuring strict adherence to infection control and safety standards
- ????️⚕️ Supervising and directing LPNs/LVNs and Home Health Aides to ensure safe, effective, and high-quality care delivery
- ???? Monitoring clinical documentation to ensure compliance with regulatory and payor requirements
- ???? Reviewing patient outcomes and ensuring continuity of care across the team

???? What We’re Looking For
- Active and unrestricted Registered Nurse (RN) license
- Experience in home health, med-surg, or acute care preferred
- Strong assessment, leadership, and documentation skills
- Ability to supervise and delegate to LPNs/LVNs and care staff
- Compassionate communicator with a patient-first mindset
- Ability to work independently and manage multiple cases in the field

Valid Auto Insurance

At least 2 years of Home Health RN experience preferred

HomeCare HomeBase experience preferred
