The Case Manager is responsible for coordinating resident care, facilitating individualized service planning, supporting communication across stakeholders, and maintaining case-related documentation in accordance with program expectations. The Case Manager functions as the central coordinator of case-related activity; this role operates within the scope of RCCF case management and in alignment with program policies and regulatory requirements.
CORE FUNCTIONS AND RESPONSIBILITIES
Care Coordination
- Coordinate services across internal teams and external providers
- Schedule and track appointments and follow-up services
- Maintain communication with placing agencies, legal stakeholders, and approved supports
- Support continuity of care across admission, placement, and discharge
Individualized Care Planning
- Facilitate development and ongoing review of individualized care plans in collaboration with the interdisciplinary team
- Track progress toward goals and communicate updates
- Ensure care plans reflect current needs, strengths, and service recommendations
- Participate in treatment team meetings and case reviews
Family and Stakeholder Engagement
- Support communication with families, guardians, and caregivers as appropriate
- Coordinate participation in meetings and planning processes
- Provide updates and coordination support within role scope
Documentation and Compliance
- Maintain accurate, timely, and organized case documentation
- Document case management activities and key communications
- Assist with required reports, summaries, and discharge documentation
- Support maintenance of complete and current resident records
Discharge and Transition Planning
- Initiate discharge planning at admission and update through placement
- Coordinate with internal and external stakeholders to support transitions
- Assist with referrals and connection to aftercare resources
- Contribute to discharge summaries and transition documentation
Team Collaboration
- Collaborate with residential, clinical, and educational team members
- Share relevant information to support consistency of care
- Participate in staff meetings, supervision, and case consultations
- Support implementation of trauma-informed and developmentally responsive practices
Scope and Role Boundaries
- Does not provide clinical therapy or independent mental health treatment
- Works under supervision of Program Director
- Collaborates with licensed clinicians and external providers
- Does not hold administrative or supervisory authority unless explicitly assigned
- Operates within RCCF regulations, organizational policies, and professional scope
Minimum Qualifications
- Bachelors degree in a related field
- 2+ years of relevant professional experience
- Experience working with adolescents and trauma
- Strong organizational and communication skills
- Ability to manage multiple cases and deadlines
Preferred Qualifications
- Masters degree in related field
- Experience in residential or child welfare settings
- Experience with trafficking survivors
- Knowledge of Colorado systems and resources
Required Credentials
- Valid drivers license
- Background check clearance
- CPR/First Aid (or obtain upon hire)
