About the Role
As a Home Health Registered Nurse, you will provide skilled nursing services to patients in their homes under the direction of the Director of Nursing and in accordance with physician orders and individualized care plans.
This position requires travel throughout Wayne and Monroe Counties and offers the opportunity to build meaningful relationships with patients while delivering high-quality, personalized care.
Key Responsibilities
Skilled Nursing Care
- Perform comprehensive patient assessments.
- Develop, implement, and evaluate individualized plans of care.
- Provide skilled nursing services in accordance with physician orders.
- Administer medications, injections, and treatments as prescribed.
- Perform wound care and dressing changes.
- Monitor patient progress and identify changes in condition.
- Educate patients and caregivers on disease management, medications, and safety.
- Coordinate care with physicians, therapists, caregivers, and other healthcare professionals.
- Respond promptly to patient needs and clinical concerns.
Documentation & Compliance
- Complete timely and accurate electronic documentation.
- Maintain compliance with Medicare, state, and agency regulations.
- Participate in case conferences and interdisciplinary team meetings as needed.
- Maintain patient confidentiality and HIPAA compliance.
Qualifications
Required
- Current Michigan Registered Nurse (RN) license in good standing.
- Minimum of one (1) year of nursing experience.
- Current CPR/BLS certification.
- Valid driver's license.
- Reliable transportation with current auto insurance.
- Excellent clinical assessment and critical thinking skills.
- Strong communication and documentation skills.
- Ability to work independently while maintaining excellent patient care.
Preferred
- Home Health experience.
- IV therapy and infusion experience.
- Wound care experience.
- Electronic Medical Record (EMR) experience.
Schedule
Flexible PRN scheduling based on your availability.
