We provide in-home care to former Nuclear Weapons Workers who are facing chronic and terminal illnesses due to their work environment. As an RN-Case Manager, you will conduct in-home nursing visits for our clients, ensuring they receive the care they need.

Your Role

In this role, you will:

• Conduct weekly nursing visits lasting 1-2 hours.
• Perform physical assessments, including vital checks.
• Ensure home safety with medical equipment and supplies.
• Fill weekly medication planners.
• Oversee skilled and non-skilled caregivers providing care in the client's home.

Qualifications

• Graduate of a state-approved school of professional registered nursing.
• BSN preferred.
• Current, unrestricted RN license in the state(s) of practice.
• Minimum of two (2) years nursing experience, including one (1) year in home care or a closely related field.
• One (1) year of supervisory and/or case management experience preferred.
• Current CPR certification.

Essential Functions

• Provide direct care and case management for assigned clients.
• Collaborate with the regional director and other personnel to enhance services.
• Plan, implement, and evaluate care provided.
• Coordinate client care conferences as needed.
• Serve as a local on-site clinical resource.
• Assist with staffing and scheduling coordination.
• Ensure adherence to Agency policies and nursing delegation guidelines.
• Participate in interviewing and training clinical personnel.
• Engage in performance improvement initiatives.
• Support marketing activities by visiting clients or physicians.

Standard Rate: $40.00 Hourly plus shift differentials, where applicable.
