Speech-Language Pathologist (SLP) – Per Diem / PRN

Job Summary

The Greek American Rehab & Care Center is seeking a compassionate and skilled Speech-Language Pathologist (SLP) to join our in-house rehabilitation team on a Per Diem/PRN basis. The SLP will provide evaluation, treatment, and care planning services to short-term subacute rehabilitation and long-term care residents with speech, language, cognitive-communication, voice, and swallowing disorders. This role offers the opportunity to work collaboratively with an interdisciplinary team dedicated to helping residents achieve their highest level of function and independence. Responsibilities are consistent with those commonly found in skilled nursing and subacute rehabilitation settings.

Responsibilities

  • Evaluate residents for speech, language, cognitive, voice, and swallowing impairments.
  • Develop and implement individualized treatment plans based on clinical findings and resident goals.
  • Provide dysphagia assessment and swallowing intervention.
  • Deliver speech, language, cognitive-communication, and voice therapy services.
  • Complete timely and accurate documentation, progress reports, discharge summaries, and care plans.
  • Participate in interdisciplinary team meetings and resident care conferences.
  • Educate residents, families, caregivers, and staff regarding treatment recommendations and safe swallowing strategies.
  • Ensure compliance with Medicare, state, federal, and facility documentation standards.
  • Collaborate with physicians, nursing staff, dietary services, physical therapists, occupational therapists, and other healthcare professionals.

Qualifications

  • Master's Degree in Speech-Language Pathology from an accredited program.
  • Current Illinois Speech-Language Pathology license.
  • Certificate of Clinical Competence (CCC-SLP) preferred.
  • Experience in skilled nursing, post-acute care, subacute rehabilitation, or long-term care preferred.
  • Knowledge of dysphagia management, cognitive rehabilitation, and communication disorders.
  • Strong communication, organization, and documentation skills.

Preferred Experience

  • Skilled Nursing Facility (SNF) or subacute rehabilitation experience.
  • Experience treating medically complex patients recovering from stroke, neurological conditions, surgery, or acute illness.
  • Familiarity with Medicare Part A and Part B documentation requirements.

Schedule

  • Per Diem / PRN
