Location: Portland, OR (West Portland and surrounding areas)
Duration: 13 weeks
Shift: Day shift, 5x8-hour shifts (8:00 AM – 4:30 PM)
Job Responsibilities

Provide skilled home health physical therapy services under the supervision of a Physical Therapist
\t
- \t
Deliver individualized treatment interventions per established plans of care
\t
- \t
Treat patients in their homes on a 1:1 basis, with up to 6 visits per day and standard productivity expectations (~30 points)
\t
- \t
Perform therapeutic exercises, mobility training, gait training, balance activities, and functional task training
\t
- \t
Assist patients with transfers, ambulation, and use of assistive devices
\t
- \t
Monitor patient response to treatment and report progress or concerns to supervising PT
\t
- \t
Complete accurate and timely clinical documentation, including OASIS-related components as applicable
\t
- \t
Educate patients and caregivers on safety, mobility strategies, and home exercise programs
\t
- \t
Travel between patient homes using personal transportation; routes are generally grouped to minimize drive time
\t

Collaborate with interdisciplinary team members to support continuity of care
\t

Required Skills & Qualifications

Minimum 1 year of skilled Home Health PTA experience
\t

State Physical Therapist Assistant license (required)
\t

BLS certification
\t
- \t
OASIS experience (required)
\t
- \t
Experience providing care in a home health setting
\t
- \t
Ability to work independently in the field
\t
- \t
Reliable personal transportation
\t
- \t
Willingness to float between facilities within the state if required
\t

Preferred Skills & Experience

Experience with Epic Home Health charting (Epic Remote Client and Rover mobile app)

Strong familiarity with home health workflows and productivity expectations
