The Care Coordinator plays a crucial role in managing staffing and scheduling to support quality patient care.

What You'll Do:
✔ Coordinate and schedule caregivers and nursing staff
✔ Match caregivers to clients based on needs and availability
✔ Ensure all shifts are fully covered
✔ Recruit and onboard new caregivers
✔ Maintain staff records and enter data in AxisCare
✔ Communicate with staff, clients, and clinical teams
✔ Stay compliant with Medicaid, Medicare, and DCH guidelines

What We're Looking For:

✔ 1+ year recruiting experience (required)
✔ Strong communication & organizational skills
✔ Ability to multitask and problem-solve quickly
✔ Knowledge of home care programs (CCSP, SOURCE, etc.) is a plus
