Physical Therapist (PT) – Assisted Living Facility | New Jersey
Salary Range: $95,000 – $120,000 per year
Location: Jackson Township,, New Jersey
Job Type: Full-Time
Schedule: Monday to Friday, 9 AM – 5 PM
We are seeking a compassionate and licensed Physical Therapist to join our team in providing on-site therapy services within assisted living communities. This role focuses on improving residents’ strength, balance, mobility, and overall function, helping them age safely and independently in place.
We are seeking a Physical Therapist to deliver outpatient-style rehab services directly within Assisted Living Facilities. This role is ideal for clinicians who are passionate about promoting aging in place and functional independence in a residential setting. You’ll work autonomously but collaboratively within each community to support resident wellness and recovery.
Key Responsibilities:
- Evaluate residents’ physical therapy needs and develop individualized care plans.
- Provide direct treatment focused on fall prevention, mobility, pain management, and ADLs (activities of daily living).
- Collaborate with facility staff, caregivers, and healthcare providers to ensure continuity of care.
- Maintain accurate and timely documentation in compliance with state and federal regulations.
- Educate residents and staff on home exercise programs and injury prevention.
Requirements
- Graduate of an accredited Physical Therapy program.
- Active New Jersey Physical Therapist license.
- Experience working with older adults or in senior living settings is preferred.
- Strong interpersonal skills and ability to communicate effectively with seniors and team members.
- Self-motivated, professional, and organized.
