About the Role
-
Community-based physician rounding in subacute skilled nursing facilities
-
Flexible scheduling - you control your day
-
Low census: 10-20 participants seen once weekly
-
Lead clinical oversight and quality for provider services
-
Collaborate with an experienced, supportive interdisciplinary team
-
Serve as a physician leader and mentor to NPs and other providers
-
Champion person-centered care for frail elderly participants

Qualifications
-
MD or DO with active state medical license
-
Board Certified in Internal Medicine or Family Medicine (geriatrics preferred)

DEA registration and ability to maintain hospital privileges

Experience in primary care with frail or elderly populations
