Position Summary: The role of the Chaplain is to ensure that each patient and/or family have spiritual care counseling services, as desired. The Chaplain develops and implements the spiritual aspects of hospice care, participates in the hospice team, and arranges the provision of spiritual and emotional care to patients, families and staff members.

Qualifications:
- Graduate of an accredited Seminary or School of Theology (M. DIV or equivalent Theological degree)
- 4 units of CPE (required, no exceptions)
- Ecclesiastical endorsement
- Board certification (preferred)
- Experience working with death and dying, individuals/family/caregivers
- Knowledge and commitment to hospice philosophy of care
- Ability to communicate effectively publicly, one-on-one, written and by phone
- Ability to provide nonjudgmental listening presence
- Ability to find creative ways to engage people with Alzheimer’s and cognitive decline
- Ability to accept different lifestyles, cultures, beliefs and values
- Ability to network with community clergy and congregations
- Ability to provide devotions in a staff setting
Duties:
- Ensure patients and/or families have spiritual care counseling services, as desired
- Is familiar with the Patient’s Care Plan and incorporates those Interventions and Goals into Patient Visits (when applicable).
- Develop a Patient Care Plan to include spiritual aspects of hospice care individualized to the patient and family
- Participate as a member of an Interdisciplinary Teams when possible
- Provide leadership and officiating of funerals when asked by families
- Actively participates with Chaplain Team to provide care for all patients, families and staff
- Communicate with assigned Chaplain after all patient/family visits (including In Person, Phone, and Electronic Communication for continuity of care
- Maintain communication with Supervisor regarding Weekly Schedule, Patient/Family Concerns and Chaplain Requests
- Attend regularly scheduled meetings
- Participate in providing spiritual care to staff through (but not limited to):
- One-on-one interaction and support, intentional and mature listening presence
- Providing prayer, devotions, Memorial meditations, etc. in all staff gatherings/meetings
- Maintain Ecclesiastical endorsement/ordination and Board Certification requirements
