Position Overview:
The Community Health Nurse (RN) provides clinical support, health education, and administrative oversight to ensure the smooth functioning of T and Ns clinical and wellness programs. This role also contributes to the organizations mission by collaborating with the team to promote equitable access to healthcare and wellness resources.
Essential Responsibilities:
Clinical Support
- Assist patients during clinic hours and help navigate complex medical needs in the office or in the community.
- Provide health education to clients on the use of medical devices such as blood pressure monitors, glucometers, and insulin pens.
- Conduct intake, initial and routine health assessments
- Maintain a weekly caseload of 15 participants, ensuring at least 13 patients are seen each week.
- Support external providers with medical case management
- Manage harm reduction programs, including needle exchange and referrals to substance use treatment.
- Follow up with patients after emergency department visits and hospital discharges.
Administrative & Risk Management
- Coordinate safety activities, including annual medical equipment inspections.
- Represent T and N in local coalitions and community partnerships to advance public health initiatives.
Qualifications:
- Active RN license in Washington, D.C.
- Strong commitment to serving diverse patient populations, particularly those impacted by health disparities and substance use.
- Demonstrates a strong work ethic and excellent time management skills to ensure timely submission of documentation.
- Ability to work collaboratively with volunteers, staff, and community partners.
- Familiarity with Google Workspace tools (Gmail, Docs, Calendar) or the ability to learn new technology systems and EMR.
