Key Responsibilities

The Home Health LPN provides skilled nursing services within the scope of the Arizona Nurse Practice Act, following the established plan of care.

Clinical Care & Procedure

  • Administer prescribed medications and treatments (oral, injectable, topical) safely and accurately.
  • Perform skilled nursing procedures, including wound care and dressing changes, catheter care, and ostomy management.
  • Monitor and document patient vital signs and overall health status, reporting any significant changes to the supervising RN or physician immediately.
  • Collect samples for laboratory tests as ordered.
  • Assist with the coordination of care with other healthcare disciplines (PT, OT, etc.) and community resources.

Patient Support & Education

  • Provide direct patient care and assistance with activities of daily living (ADLs) as needed.
  • Educate patients and family members on disease management, safe medication usage, and self-care techniques to foster independence.
  • Offer emotional support and companionship, recognizing the psychological and social aspects of recovery and chronic illness.
  • Ensure a safe home environment, performing fall and safety assessments.

Documentation & Compliance

  • Maintain accurate, complete, and timely clinical records and documentation using our electronic health record (EHR) system.
  • Adhere strictly to all Foundation Home Health policies, procedures, and state/federal regulatory guidelines (e.g., Medicare/Medicaid).

✅ Qualifications

Required

  • Current, valid, and unrestricted Licensed Practical Nurse (LPN) license in the state of Arizona.
  • Current Basic Life Support (BLS) certification.
  • A minimum of one (1) year of professional nursing experience, with home health, hospice, or medical/surgical experience preferred.
  • Reliable transportation, a valid Arizona Driver's License, and current auto insurance, as travel between patient homes is required.
  • Strong critical thinking, organizational, and time-management skills.
  • Excellent communication and interpersonal abilities for working with patients, families, and the healthcare team.

Preferred

  • Prior experience with electronic medical record (EMR) systems, specifically in a home health setting.
