WellTech Partners is working with a few skilled nursing facilities in Santa Monica that are seeking full-time, part-time and PRN therapists. We are looking for hard-working, team oriented and reliable PT's that value patient care and exhibit exceptional clinical excellence.

Key Responsibilities:

• Evaluate patients to develop individualized treatment plans in accordance with physician orders, SNF standards of care and current levels of evidence to most favorable positive patient outcomes.

• Provide therapeutic interventions focused on mobility, pain reduction, strength, balance, and overall functional improvement.

• Collaborate with an interdisciplinary care team including OT, SLP, nursing staff, and social services.

• Educate patients, families, and caregivers on home exercise programs and continued care strategies.

• Participate in care planning meetings and contribute to quality improvement initiatives.

Qualifications:

• Doctor of Physical Therapy (DPT) or Bachelor's/Master’s degree in Physical Therapy from an accredited program.

• Current and valid Physical Therapy CA license.

• BLS certification (or willingness to obtain upon hire).

• Strong clinical reasoning and communication skills.

• Experience in geriatrics or SNF settings preferred, but **new grads are welcome to apply**
