Care Management Support Coordinator II

Location: Remote (San Antonio, Texas Residents Only)

Pay: $17/hr

Schedule: Monday through Friday, 8:00 AM to 5:00 PM

Dean's Professional Services is actively hiring a Care Management Support Coordinator II to support administrative care management operations in a fully remote environment. This is a great opportunity for candidates with strong customer service or call center experience who are looking to grow their career in healthcare while working from home.

In this role, you will support members through outreach, inbound calls, scheduling services, and documentation. You will serve as a key point of contact for members, providers, and internal staff while ensuring all member records are maintained in compliance with state and regulatory guidelines.

What You Will Do

• Perform outbound member outreach and handle inbound calls.
• Assist members with scheduling services and care plan next steps.
• Serve as first-line support for member and provider inquiries.
• Support new member onboarding, including sending welcome letters and educational materials.
• Maintain accurate, non-clinical member documentation in compliance with regulations.
• Connect members to health plan benefits, community resources, and SDOH referrals.
• Apply knowledge of assigned health plans and available resources.
• Comply with all company policies, procedures, and quality standards.

What We Are Looking For

• High School Diploma or GED required.
• One to two years of customer service or call center experience required.
• Bilingual is preferred but not required.
• Microsoft Excel experience is preferred.
• Strong communication, organization, and documentation skills.
