Position Function Summary:
Are you passionate about making a difference in students' lives? Do you have a knack for navigating complex systems and connecting people with the resources they need? If so, we have the perfect opportunity for you! As a School-Based Behavioral Health Patient Navigator (BH-PN) at Hawaiʻi Island Community Health Center (HICHC), you'll play a crucial role in assessing students' needs, developing service plans, and collaborating with community resources to ensure students receive the best care possible.

Essential Duties and Responsibilities:
Your days will be filled with exciting and meaningful tasks, including:

???? Screening new service requests and ensuring students are served according to their individual needs.
???? Working directly with students to reduce barriers and address social determinants of health (SDOH).
???? Educating students on SDOH and helping them navigate various systems.
???? Developing treatment/service plans and establishing alliances with students.
???? Providing ongoing support and case management to help students achieve their goals.
???? Participating in specialized support services like Education and Wellness groups.
???? Collaborating with school staff to integrate health education and social-emotional wellness programming.
???? Engaging in interagency planning and service coordination to improve service continuity.

Position Specifications:
To thrive in this role, you'll need:

???? A Bachelor's degree in a relevant human services field plus at least one year of experience, or an equivalent combination of experience, training, and education.
???? Previous experience working in a team environment and knowledge of the Hawaiʻi Island community.
???? Eligibility for future licensure in Social Work, Counseling, or a related field.
???? Strong social skills, problem-solving abilities, and leadership qualities.
???? Excellent communication skills and the ability to work as part of an interdisciplinary team.
???? Physical stamina to stand, walk, lift up to 50 pounds, and perform other physical tasks.
