Position Function Summary:
Are you passionate about making a difference in students' lives? Do you have a knack for navigating complex systems and connecting people with the resources they need? If so, we have the perfect opportunity for you! As a School-Based Behavioral Health Patient Navigator (BH-PN) at Hawaiʻi Island Community Health Center (HICHC), you'll play a crucial role in assessing students' needs, developing service plans, and collaborating with community resources to ensure students receive the best care possible.

Essential Duties and Responsibilities:
Your days will be filled with exciting and meaningful tasks, including:

  • ???? Screening new service requests and ensuring students are served according to their individual needs.
  • ???? Working directly with students to reduce barriers and address social determinants of health (SDOH).
  • ???? Educating students on SDOH and helping them navigate various systems.
  • ???? Developing treatment/service plans and establishing alliances with students.
  • ???? Providing ongoing support and case management to help students achieve their goals.
  • ???? Participating in specialized support services like Education and Wellness groups.
  • ???? Collaborating with school staff to integrate health education and social-emotional wellness programming.
  • ???? Engaging in interagency planning and service coordination to improve service continuity.

Position Specifications:
To thrive in this role, you'll need:

  • ???? A Bachelor's degree in a relevant human services field plus at least one year of experience, or an equivalent combination of experience, training, and education.
  • ???? Previous experience working in a team environment and knowledge of the Hawaiʻi Island community.
  • ???? Eligibility for future licensure in Social Work, Counseling, or a related field.
  • ???? Strong social skills, problem-solving abilities, and leadership qualities.
  • ???? Excellent communication skills and the ability to work as part of an interdisciplinary team.
  • ???? Physical stamina to stand, walk, lift up to 50 pounds, and perform other physical tasks.
