Cordial Health is seeking a dedicated and compassionate Psychiatry Nurse Practitioner to join our team. We are a leading healthcare organization providing comprehensive mental health services to patients in nursing homes across Plattsburgh, NY. Responsibilities:
- Conduct comprehensive psychiatric evaluations and assessments of elderly patients in nursing homes.
- Diagnose and develop individualized treatment plans based on the patient's mental health needs.
- Provide medication management and adjust treatment plans as necessary.
- Collaborate with interdisciplinary teams, including physicians, psychologists, and social workers, to ensure holistic patient care.
- Offer counseling and psychotherapy services to patients and their families.
- Educate patients and their families about mental health conditions, treatment options, and self-care strategies.
- Maintain accurate and up-to-date medical records in compliance with organizational and regulatory standards.
- Participate in clinical meetings, case conferences, and quality improvement initiatives.
Requirements:
- Valid Nurse Practitioner license in the state of New York with specialization in Psychiatry.
- Ability to work independently and effectively manage a caseload of patients.
- Strong diagnostic skills and knowledge of psychiatric medications and treatment modalities.
- Excellent interpersonal and communication skills to establish rapport with patients, families, and interdisciplinary team members.
- Proficient in electronic health record (EHR) systems and basic computer skills.
- Compassionate, patient-centered approach to care.
