8:00 AM - 4:30 PM 8 Hr Day 40 RN only - Case Manager position Start Date: immediately Monday - Friday 8a - 4:30p, no wknds 40 hours/week 12 weeks duration Training 1 week orientation and shadowing 1 year Home Health exp. Required Experience with Point Care and Homecare Homebase Software is required Should be comfortable performing Oasis visits independently and confidently Will use a web based tablet A number of visits on in Pres Home senior living communities Average milage 20 to 50 miles daily
