Job DescriptionPosition Summary
The RN Clinical Manager — Private Duty Nursing (PDN) is a senior registered nurse leader responsible for the clinical oversight, quality assurance, regulatory compliance, and day-to-day management of a private duty nursing program serving pediatric and/or adult patients in their homes. This role is specifically focused on the unique demands of PDN cases — including technology-dependent patients, ventilator management, tracheostomy care, complex pediatric cases, and extended-hour nursing shifts — and requires a nurse leader with deep clinical expertise in high-acuity home-based care. The RN Clinical Manager PDN ensures that every patient receives safe, individualized, skilled nursing care delivered by a well-supervised, competent clinical team, in full compliance with the New York State Nurse Practice Act, NYS Department of Health regulations, and Medicaid/managed care requirements. The ideal candidate is a confident clinical leader who is equally at home reviewing a care plan, conducting a field supervisory visit, mentoring a bedside nurse, and presenting outcomes data to agency leadership.
Key ResponsibilitiesPDN Clinical Oversight & Patient Care Management
- Maintain overall clinical accountability for the safety, quality, and appropriateness of all private duty nursing care delivered across the agency's PDN caseload
- Oversee the clinical management of complex, technology-dependent patients including those requiring ventilator support, tracheostomy care, G-tube/J-tube/NG-tube feedings, IV therapy, continuous oxygen, cardiac monitoring, and wound care
- Review, approve, and monitor individualized Plans of Care (POCs) for all PDN patients — pediatric and adult — ensuring alignment with physician orders, patient needs, and payer authorizations
- Conduct or oversee initial nursing assessments and start-of-care evaluations to establish clinically appropriate, comprehensive care plans prior to first visit
- Perform and document supervisory nursing visits in the patient's home to directly observe and evaluate the quality of PDN care being delivered
- Lead clinical case conferences for high-acuity, complex, or unstable patients involving physicians, specialists, therapists, families, and nursing staff
- Respond to clinical escalations, patient safety events, and emergencies with sound, timely clinical judgment
- Oversee discharge planning for PDN patients transitioning to lower levels of care, inpatient settings, or other providers
- Ensure new PDN admissions have a completed nursing assessment and start-of-care visit within required timeframes.
Qualifications
Required
- Active, unrestricted Registered Nurse (RN) license issued by the New York State Education Department (NYSED) — required
- Bachelor of Science in Nursing (BSN) required; Master of Science in Nursing (MSN) strongly preferred
- Minimum 5 years of clinical nursing experience, with at least 3 years in private duty nursing, pediatric home care, or high-acuity home-based nursing
- Minimum 2-3 years of nursing supervisory or management experience in a home care or PDN setting
- Demonstrated clinical competency in ventilator management, tracheostomy care, G-tube/J-tube/NG-tube care, IV therapy, and other complex nursing skills common in PDN
- In-depth knowledge of NYS DOH LHCSA regulations (10 NYCRR Part 763), NYS Medicaid, MLTC, and private duty nursing authorization processes
- Strong leadership, staff development, and team management skills
- Excellent written and verbal communication and critical thinking skills
- Current BLS/CPR certification (American Heart Association); PALS preferred for pediatric PDN programs
Other Requirements
- Satisfactory background check and fingerprinting in accordance with NYS Social Services Law
- Valid New York State driver's license and reliable transportation for field supervisory visits
