- Job Requirements
- Must have BLS certification (American Heart Association).
- License/Certificate verification must be dated within 30 days of start.
- Fit Test required.
- Trinity 10 Panel Drug Screen completed within 30 days of start date, preferred within 14 days, with MRO signature.
- Trinity Code of Conduct Form must be signed.
- Standard i9 completed by the agency.
- Confidentiality and Security Agreement signed.
- Integrity and Audit Services Form signed prior to start.
- Non Employee Upload Form completed.
- Trinity Specialty Specific Competency Testing completed within a year.
- Color Vision test required.
- COVID-19 Vaccine proof or declination/exemption required.
- MMR vaccination proof or IgG titer required.
- Hepatitis B vaccination series required; titers not accepted.
- Influenza Vaccine proof or declination/exemption required.
- 2 Step PPD OR IGRA (Quantiferon/Tspot) required.
- Varicella vaccination proof or IgG titer required.
- Physical/Health Statement within last year required.
- Trinity Profile including resume, skills checklist, and references required.
- Conflict Of Interest Disclosure Form signed.
- Criminal Background Check within last 7 years, dated within 30 days of start.
- Education Verification required.
- EPLS report dated within past 30 days required.
- GSA/OIG exclusion check dated within past 30 days required.
- Joint Commission CORE Exam within a year required.
- NSO report dated within 30 days of start required.
- System Access Form completed one week before start.
- Skills Checklist provided by agency for clinical contractors.
- Tdap vaccine or declination accepted if no pediatric interaction.
- Child and Adult Dependent Abuse Training (Iowa only).
- Iowa Sing Report (Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries) within 30 days prior to start.
- OFAC sanctions check completed within 30 days of start.
- Schedule Information
- 4 10-hour shifts, 6:30 AM to 5:00 PM.
- Every 4th weekend shift required.
