We’re Hiring! Join our team as a Patient Care Coordinator with a focus on Chronic Disease Management.
Are you passionate about helping patients take control of their health and improve their quality of life? We’re looking for a dedicated Patient Care Coordinator to join our Valley County Health System (VCHS) team. This is full-time position working 5 days per week.
JOB SUMMARY:

<li>Provide education and support to patients living with chronic conditions such as diabetes, heart disease, COPD, and hypertension</li>
<li>Develop individualized care plans</li>
<li>Monitor patient progress and outcomes</li>
<li>Collaborate with interdisciplinary healthcare teams</li>
<li>Empower patients to build sustainable, healthy habits</li>
<li>Provide direct patient care within the medical clinic</li>
</ul>
REQUIREMENTS: The ideal candidate is compassionate, organized, able to be a self-starter, can work independently, and is committed to patient-centered care. Experience in care coordination, patient education, and chronic condition management is preferred.
Applicants must have current Nebraska licensure as a Registered Nurse.
