Responsibilities

About the Role

During visits that may last up to one hour, our providers assess and observe a member’s current health status, medical history, medication adherence, social circumstances, and other potential risks. This approach provides invaluable insights into a member’s overall health that are often missed during standard office visits. The Matrix Comprehensive Health Assessment (CHA) enhances care quality and allows us to address multiple care gaps in a single appointment. Our providers are responsible for:

  • Conducting assessments for adults and geriatrics, including medical history, diagnosis and treatment, health education, referrals to physicians and case management, follow-up, and thorough documentation in accordance with our guidelines.
  • Administering and ordering necessary point-of-care and lab-based tests.
  • Collaborating with physicians, case managers, social workers, family members, key caregivers, and other medical personnel as appropriate.
  • Working alongside Primary Care Physicians (PCPs) to provide patient education and follow-up as needed.
  • Delivering services at the member’s residence, which may include their homes or Skilled Nursing Facilities.

Qualifications

Skills & Experience That Shine

  • A Master’s Degree or equivalent experience along with successful completion of NP licensure.
  • Current NP licensure in the state of practice, including prescription authority or the ability to obtain it as necessary.
  • AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
  • Current BLS, ACLS, or CPR certification.
  • Preferred one year of NP experience; however, new graduates are encouraged to apply!
  • Willingness to travel up to 15% of the time (including overnight travel) to various locations as needed.
  • A valid state driver’s license and proof of adequate automobile insurance for the state of residence.
  • Strong computer skills and familiarity with health/medical record software.
  • Excellent verbal and written communication abilities.
