Responsibilities:

Develop a comprehensive social history, social assessment, and care plan for each new resident, identifying areas of need and implementing effective strategies for resolution

Collaborate with residents, families, and interdisciplinary teams to ensure individualized care and support

Provide counseling and support to residents and families, promoting positive relationships and effective communication

Participate in care plan meetings, evaluating the effectiveness of interventions and revising plans as needed

Assist with resident discharge and placement planning, ensuring a smooth transition to the next stage of care
Requirements:

Currently licensed social worker per State of Nebraska requirements

Minimum of two (2) years social work experience in a healthcare-related field

Ability to maintain regular attendance and complete Social Services Certification through The State of Nebraska

Must possess computer skills, good organizational and time management skills, and the ability to maintain confidentiality

Excellent communication and interpersonal skills, with the ability to work cooperatively with residents, families, and staff
