Job Title: RN – Case Manager
Job Location: Syracuse, NY 13215
Duration: 13 weeks (extendable)
Shift: 9:00 AM – 5:30 PM
Pay Rate: $60 – $65 per hour
Position Summary:
The RN Case Manager is responsible for coordinating and managing patient care across the continuum in an acute care hospital setting. This role focuses on discharge planning, utilization review, care coordination, and ensuring high-quality, cost-effective patient outcomes.
Key Responsibilities:

Coordinate patient care and discharge planning in an acute care hospital environment.

Collaborate with physicians, nursing staff, social workers, and multidisciplinary teams to develop and implement care plans.

Conduct utilization review and ensure appropriate level-of-care determinations.

Facilitate safe and timely patient transitions to the next level of care.

Communicate effectively with patients, families, and healthcare providers regarding care plans and post-discharge needs.

Maintain accurate and timely documentation within the electronic medical record system.

Ensure compliance with hospital policies, regulatory standards, and payer requirements.

Participate in interdisciplinary rounds and case conferences as needed.

Advocate for patients to ensure access to appropriate services and resources.

Perform other related duties as assigned.
Required Skills:
-
Experience with EPIC Electronic Medical Records.
-
Acute care hospital case management experience.
Qualifications:
-
Current, active New York State RN license.
-
BLS certification (must be current and able to be recertified).

Minimum of 1.5 years of recent experience (within the past 2 years) in an acute care facility of similar size and complexity.

Travelers must have experience in hospitals of equal or greater size or trauma centers.

Proven experience using EPIC EMR.

Strong communication, organizational, and care coordination skills.
