Title: Patient Care Coordinator (RN) License: Active RN license from any state of the US is accepted Place of Work (City/State): Northern Navajo Medical Center, Shiprock, NM Start Date: March 2026 Duration: Options with 6 months contract and extendable Shift Hours: (Inpatient and outpatient): 8 hours, 10 hours or 12 hours of shifts Rotating of days, evenings, nights and weekends including Holidays. How many shifts needed per month: 20-22 Shifts a month Experience needed: A minimum of 3 years of Nursing experience with a minimum of 2 years in tour coordination Certifications: Active BLS and ACLS Special Skills needed: Coordinates callback for support departments, i.e. Facilities, Bio-Med, Laboratory, Radiology, Operating Room staff, Motor Vehicle Operators, and Medical Staff. Credentialing / Onboarding time: 6 to 8 weeks of onboarding (background checks) Responsibilities:

• Coordinate patient care activities across departments to ensure smooth hospital operations during assigned shifts.
• Manage and document patient admissions, discharges, and intra/interfacility transfers using the Electronic Health Record system.
• Conduct nursing rounds to assess patient care needs, staffing, safety, and environmental concerns, escalating resources as needed.
• Support emergency situations by assisting with patient care, monitoring patient conditions, and communicating critical observations to physicians
