Degree in Physical Therapy, active Physical Therapist license and 1+ year of Home Health Physical Therapy experience required. Applicants who do not meet these qualifications will not be considered.
A physical therapist with expertise in home health is needed to provide high-quality care within a defined group of zip codes in the Washington, DC area. This contract role requires proficiency in Homecare Homebase and OASIS documentation and involves coordinating visits to patients homes while maintaining reliable transportation and up-to-date licensure.
Location
This position serves the Naval Anacost Annex area in Washington, DC. Contractors will cover assigned zip codes within the District based on staffing demands. Mileage expenses from the first to the last patient visit of each day are reimbursed. Candidates should be located within commuting distance or be open to traveling as needed.
Responsibilities
- Deliver skilled physical therapy services in patients homes, adhering to care plans and clinical guidelines
- Accurately document patient encounters using Homecare Homebase and OASIS platforms
- Coordinate with interdisciplinary teams and report patient progress
- Maintain compliance with state licensure, BLS certification, and agency policies
- Provide coverage by floating to alternate zip codes when requested
Qualifications
- Valid medical license for the District of Columbia
- Minimum of 2 years of home health physical therapy experience
- BLS certification required
- Experience with Homecare Homebase and OASIS documentation mandatory
- Reliable personal vehicle meeting all legal and insurance requirements
Schedule
- Monday through Friday, 8:00 AM to 5:00 PM
