Position Summary
Unity Health Care is seeking a Full-Time Nurse Practitioner to join our mission-driven team. Under the supervision of the Medical or Specialty Director, the Nurse Practitioner is responsible for delivering comprehensive, high-quality, patient-centered care in a community-based setting. This includes diagnosing and treating physical and behavioral health conditions, counseling and educating patients, and collaborating with an interdisciplinary team to support population health outcomes.
Key ResponsibilitiesHealth Outcomes

Conduct clinical histories, physical exams, and assessments.

Order and interpret diagnostic tests and imaging.

Develop and implement treatment plans using evidence-based guidelines.

Provide preventive care, education, and chronic disease management.

Refer patients to specialists, hospitals, and community services as needed.

Participate in quality improvement initiatives and clinical best practice reviews.

Maintain accurate and timely electronic documentation.

Collaborate with health center leadership on panel management and safety reporting.
Patient Experience

Deliver culturally competent and respectful care to a diverse population.

Partner with patients in shared decision-making and care planning.

Communicate clearly and compassionately; use interpretation services when needed.

Ensure timely follow-up on results, phone calls, and care coordination.

Maintain a consistent clinical schedule that supports access to care.

Respond to patient concerns and participate in service recovery efforts.
Care Team Collaboration

Foster a collaborative, team-based environment.

Support staff development and coach team members to work at top of scope.

Participate in student and resident teaching as requested.

Attend and engage in staff, team, and organization-wide meetings.

Promote wellness, professionalism, and respectful communication.
Financial Sustainability

Monitor productivity and utilization targets with clinic leadership.

See patients efficiently while maintaining quality and safety.

Complete all documentation and claims corrections within established timeframes (e.g., notes locked within 48 hours).

Assist with population health efforts and patient panel outreach.
Minimum Qualifications

Current DC Nurse Practitioner license
-
Active DC CDS and DEA registrations (or eligibility to obtain)

Graduate of an accredited Nurse Practitioner program

Board certified or board eligible (must obtain certification within one year of hire)

Willingness to work evenings or weekends as needed for patient coverage
