Hospice Social Worker (LMSW)

Make a meaningful impact by supporting patients and families through every stage of the end-of-life journey. We are seeking a compassionate and experienced Hospice Social Worker to provide psychosocial support, counseling, care coordination, and community resource assistance for hospice patients and their loved ones.

Key Responsibilities:

• Complete comprehensive psychosocial assessments within 5 days of hospice admission.
• Identify and address emotional, social, financial, environmental, and community resource needs.
• Provide counseling and support related to end-of-life issues, grief, loss, and bereavement.
• Assist with advance directives, funeral planning, discharge planning, and family counseling.
• Collaborate with the interdisciplinary team to develop and coordinate individualized patient care plans.
• Participate in IDG meetings and maintain accurate clinical documentation.
• Advocate for patients and families while promoting dignity, respect, and quality of life.

Qualifications:

• Master's Degree in Social Work from an accredited program.
• Current Social Worker license required.
• Minimum 1 year of hospice experience required.
• Strong knowledge of end-of-life care, grief support, and community resources.
• Valid Driver’s License and reliable transportation with current auto insurance.
• Current AHA CPR Certification.
• HomeCare HomeBase experience preferred.
