Position Summary

We are seeking a compassionate and experienced Clinician (Social Worker or Nurse) to visit clients across various facilities and ensure their overall well-being. This role focuses on assessing patient needs, monitoring their physical and emotional health, and ensuring appropriate services and supports are in place. The ideal candidate is patient-centered, organized, and comfortable working independently while collaborating with interdisciplinary teams.

Key Responsibilities

  • Conduct regular on-site visits to patients in various facilities (such as nursing homes, assisted living facilities, community)

  • Assess patients’ physical, emotional, and psychosocial well-being

  • Ensure patients’ needs are being met, including medical, social, and supportive services

  • Identify concerns or changes in condition and communicate findings to appropriate care teams

  • Advocate for patients to ensure quality care and appropriate resources

  • Coordinate with facility staff, families, and healthcare providers as needed

  • Maintain accurate and timely documentation in accordance with organizational and regulatory standards

  • Follow all confidentiality, safety, and compliance guidelines

Qualifications

  • Licensed Social Worker (LCSW, LMSW, or equivalent) or Licensed Nurse (RN or LPN)

  • Current, unrestricted license in New York State

  • Experience working with vulnerable populations, long-term care, behavioral health, or community-based settings preferred

  • Strong assessment, communication, and interpersonal skills

  • Ability to work independently and manage a flexible schedule

  • Reliable transportation and willingness to travel between facilities

  • Basic computer skills for documentation and reporting

Preferred Skills

  • Knowledge of care coordination and patient advocacy

  • Experience with interdisciplinary teams

  • Bilingual abilities a plus
