Job DescriptionKey Responsibilities
- Administer prescribed medications and treatments accurately and in a timely manner
- Perform routine nursing procedures including wound care, tube care, and injections
- Monitor residents’ conditions and promptly report changes to the RN or physician
- Document all care, treatments, and observations in the electronic medical record (EMR)
- Respond to call lights, resident needs, and emergency situations as appropriate
- Collaborate with CNAs and other team members to ensure continuity of care
- Participate in shift report and communicate important updates on resident status
- Assist with admissions, discharges, and transfers
- Maintain cleanliness, organization, and security of the medication cart
- Follow infection control protocols and all facility policies and procedures
Qualifications
- Current, active LPN license in good standing
- Previous experience in long-term care or skilled nursing preferred
- Strong clinical, documentation, and communication skills
- Ability to prioritize tasks and manage time effectively
- Commitment to providing compassionate, resident-centered care
