- The Clinical Manager will coordinate and oversee all direct care patient services provided by clinical personnel.
- You will conduct assessment and reassessment of patients, update plan of care, evaluate patients' needs and adhere to LeeKO's policies and procedures.
- Monitor and coordinate caregivers' assignments.
- Review referrals for acceptance ensuring patients meet admission criteria
- Admit patients and assigns appropriate clinicians based on the patient's needs
- Effectively encourages high performance of service delivery and by being available to clinicians during business hours
- Ensure documentation is in compliance with regulatory agencies and requirements of third-party payers
- Ensures final audits/billing are completed timely and in compliance with Medicaid/Medicare and third party payers regulations
- Coordinates communication between team members/attending physicians/caregivers to ensure the appropriateness of care and outcome planning
- Conduct interview and assist with on-boarding of newly hired clinicians
- Conduct annual performance evaluation
- Conduct QAPI and annual CEMP review
- Performs other related duties as requested by upper management
Requirements
- Graduate of an accredited School of Nursing
- Current state or multi-state license as a Registered Nurse
- Current CPR.
- Valid driver’s license, with reliable transportation.
- Two years as a Registered Nurse in a home care
- Preferred at least one year of home health management experience
