The Case Manager II - Developmental Disability Services is responsible for the delivery of person-centered conflict free case management services to individuals with intellectual/ developmental disabilities and their families in accordance with Agency Vision, Mission, and Values and applicable program contracts, manuals, and guidelines. This position assists individuals in Champaign County with intellectual/developmental disabilities and their families/guardians access individualized services and supports to ensure that the services and supports are responsive to the unique needs and desires of everyone, while effectively promoting independence and self-determination. The Case Manager II is responsible for service eligibility determinations, case management of assigned cases, and monitoring the general health, safety and well-being of individuals with disabilities, including providing technical assistance to determine the appropriate support for individuals, guardians, and providers working through a variety of service issues. This position is also responsible for assisting individuals with intellectual/developmental disabilities that are nearing exit from secondary education (and their family) to identify support & services needed for adult life.
- Bachelor’s degree in human services or related field
- Have at least one year of experience providing direct services to persons with developmental disabilities
- Must obtain Qualified Intellectual Disabilities Professional (QIDP) credential within 3 months from hire
- Ability to pass criminal background check
- Valid driver’s license and ability to transport clients and visit their homes throughout service area on a routine basis using an agency vehicle
- Strong background in customer services and human services with program’s target population
- Ability to follow program procedures while adjusting work style to meet the needs of the client
- Understands steps to implement to work toward gainful employment for individuals lacking employment history
- Knowledge of public assistance programs, including eligibility and application process
- Ability to handle program and client information in a confidential manner and report concerns as mandated
- Knowledge of trauma-informed care principles
- Ability to complete timely and accurate documentation
- Proficient in English (spelling, punctuation, and grammar)
- Ability to communicate effectively both written and orally
- Computer skills needed to complete case documentation, data entry, and time sheets
- Ability to attend program meetings and/or training as required
- Ability to work a flexible schedule to accommodate client scheduling needs including periodic evenings and weekends
Schedule: 8:00 a.m. – 4:30 p.m., M-F/Flexibility is necessary to support the goals of the program.
Worksite: Flexible: office-based or hybrid (part home-based; part office-based). Probation period will be primarily conducted in office-based setting.
The onsite worksite is located at 1776 E. Washington Street, Urbana, IL 61802.
Status: Full-Time/37.5 hours per week
Starting Wage: $22.06 - $23.50
