Bachelor of Science in Nursing (BSN) degree, active Nurse Practitioner (NP) license and minimum 1+ years Nurse Practitioner (NP) experience required. Applicants who do not meet these qualifications will not be considered.

There is an open Part Time Nurse Practitioner (NP) Home Health Flexible Schedule job in Billings, MT. The Part Time Nurse Practitioner (NP) will conduct home health risk assessments within Yellowstone County, MT. The average home health risk assessment will take about 45 minutes and the nature of the visit may vary and could include an annual health assessment or post discharge visit. The Part Time Nurse Practitioner (NP) will assess for gaps in care forpreventative measures and provide digital recommendations to the primary care physician for gap closure. The home health risk assessment will be conducted on Medicare Advantage Plan members residing in Billings, MTand the surrounding Yellowstone County, MT.Yellowstone County, MT includes locations like Laurel, Shepherd, and Worden. Candidates could be scheduled in any one of these locations during an individual shift.

Responsibilities:

• Conduct annual home health risk assessments for a Medicare Advantage Health Plan
• Evaluate prior and current medical history, medical conditions and medication history
• Conduct point of care testing including fingerstick and urine testing
• Provide education on medical conditions and medications
• Identify any gaps in care and provide recommendations for resolution
• No creation of treatment plans or prescribing of medications is required

Work Hours:

• Flexible shifts between Monday and Sunday
• Create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Preferred Experience:

• 1 year of current NP experience is preferred
• Previous in home care experience is preferred, but not required

Preferred Qualifications:

• Nurse Practitioner License in MT
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC

Education Requirements:

• MSN degree from an accredited school of nursing
