Optimal Care is building a talent pipeline of compassionate Registered Nurses for future Hospice RN Case Manager opportunities serving West Michigan. Bring your nursing expertise to one of healthcare's most meaningful specialties. As a Hospice RN Case Manager, you'll provide skilled nursing care and compassionate support to patients and families during life's final chapter, helping ensure comfort, dignity, and quality of life. 

About the Role

This role is perfect for a registered nurse who wants to practice the full scope of nursing while building deep, meaningful relationships. This position will have you building report with patients, families, physicians, facilities, and other interdisciplinary team members. You'll have the autonomy to coordinate comprehensive care for your caseload while working collaboratively with our interdisciplinary team to address the physical, emotional, and spiritual needs of those in your care.

This role is a high travel position and requires daily travel within the assigned service area to provide care to patients in their homes, assisted living facilities, or nursing homes.

Service areas may include:

  • Kent
  • Muskegon / Ottawa
  • Newaygo / North Kent
  • Ionia / Montcalm / Mecosta
  • Oceana

Flexibility to work across adjacent counties within the service area is required.

Schedule

Full Time: Monday-Friday, 8:00 AM - 5:00 PM, with weekend rotation

What We're Looking For

Required Qualifications

  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing
  • Current State License as a Registered Nurse
  • Minimum one (1) year experience as a nurse in a clinical care setting
  • Reliable transportation, valid driver's license, and valid automobile insurance coverage

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN)
  • Experience in hospice or home health environment

Essential Skills and Competencies

  • Nursing skills as defined by accepted nursing standards
  • Ability to assess and respond to the needs of patients, families, and caregivers in varied settings and function effectively in home or nursing facility environments
  • Strong time management skills with ability to handle multiple priorities and deadlines
  • Demonstrates kill in nursing practice, ability to cope with family emotional stress and respect individual lifestyles
  • Ability to handle multiple priorities, documentation requirements and deadlines
  • Meets all applicable agency policies and procedures related to health screenings and required testing
