Job DescriptionThe Recollections Co-Coordinator will work alongside our Clinical Coordinator for the planning, developing, overseeing, and implementation of specialized care and services for residents with Alzheimer’s, dementia, and other memory-related conditions within the Assisted Living Community. This role’s primary responsibility will focus on the social model of the neighborhood as well as co-supervision and management of the frontline staff. The Recollections Co-Coordinator must show high levels of empathy and be able to provide the necessary support to families as they navigate the Dementia process.

Responsibilities
Resident Care and Advocacy
- Coordinate with the Recollections Clinical Coordinator and caregivers to maintain & ensure person-centered care plans are developed and being followed
- Host quarterly care conferences with families
- Serve as a primary point of contact for families
Staff Training & Supervision
- Train, mentor, and provide support to staff regarding dementia best practices
- Promote a culture of empathy, respect, and professionalism
- Ensure staff compliance with established memory care protocols and safety guidelines
Compliance & Documentation
- Maintain accurate documentation related to resident care, programming, and family communications
- Ensure compliance with all state and local regulatory agencies in addition to company policies and procedures related to memory care
* Any other task as assigned by the Administrator or President.
Candidate Requirements
Essential
- Must be able to read, write, understand and communicate effectively in English and interact tactfully with elderly residents and their families.
- High School diploma is required
- Flexibility and Professionalism
Desirable
- Two (2) year experience working in a Memory Care management role is preferred
- Proficient in PCC electronic health record system, Microsoft Office programs, and the Google Suite is preferred
- Familiar with Montessori Methods of Activity structure for a Memory Care population
Requirements
Must be at least 21 years of age
High school diploma or equivalent
Bachelors degree in Healthcare Administration (preferred)
