Job Description

We are seeking an experienced Nurse Practitioner to provide comprehensive primary care in Skilled Nursing Facilities, LTC, and ALFs.

• Perform patient assessments, exams, and chart reviews.
• Manage acute and chronic conditions, order diagnostics, and develop treatment plans.
• Prescribe medications following guidelines.
• Educate patients, families, and staff on health and prevention.
• Collaborate with care teams to ensure seamless care and support quality improvement.

Qualifications include an active Minnesota NP license, board certification, and 1+ year experience preferred.
