JOB TITLE: LICENSED SOCIAL WORKER
The Social Worker position is part of the Social Services unit. This position is responsible for the intake of clients into designated permanent housing sites, including conducting housing interviews and workshops, managing multiple reports, and coordinating with referral sources and partner agencies. This person will also participate in and contribute to a program culture that is client centered, outcome oriented, and dedicated to continuous quality improvement.
OVERVIEW OF POSITION:
• Carry and manage an active caseload of 16 residents and their families in the Rapid Rehousing Program
• Conduct comprehensive intake assessments, biopsychosocial evaluations, and individualized housing stability plans for each family upon program entry.
• Familiarity with HMIS, PADMISSION, or comparable case management software to enter case notes, facilitate transfers, and exit families into permanent housing.
• Schedule apartment viewings to house residents through proactive housing search support, landlord engagement, and rental assistance coordination.
• Provide trauma informed, survivor centered individual counseling and crisis intervention services to residents using evidence-based frameworks.
• Maintain accurate, timely, and complete case records in compliance with agency policies, funding requirements, and regulatory standards.
• Plan, develop, and facilitate evidence based in-service Life Skills Workshops and group programming for DV program residents on a regularly scheduled basis.
• Other duties as assigned and/or required by the Program Manager
NECESSARY SKILLS:
• Knowledge of community resources, associations and organizations addressing the issues of the population
• Good written and verbal communication skills and excellent presentation skills
• Familiarity with signs and symptoms of drug and/or alcohol dependency
• Must be culturally competent and sensitive towards diverse ethnic groups, cultural groups, and populations
• Ability to direct and prioritize own work and be flexible
• Knowledge of Microsoft Office Suite, HMIS, and various data base software
• Knowledge of homeless and housing system and the ability to enhance understanding to maximize the level of service delivery
QUALIFICATIONS:
• Master’s degree of Social Work from an accredited institution required
• Licensed Social Worker (LSW) in the Commonwealth of Pennsylvania required
• Minimum of two years’ experience of clinical social service experience with DV survivors, housing providing services to at-risk and under-served populations required
