Position Overview

NJ Home Health Care is seeking an Experienced Home Care Coordinator to manage a dynamic caseload, ensure exceptional customer service, and support our field staff. This role is ideal for someone who has already worked in home care coordination/scheduling and understands EVV, intake, onboarding, and day-to-day office operations.

This is not an entry-level role. We are looking for a coordinator who can step in with confidence and contribute immediately.

What You’ll Do

Scheduling & Case Management

  • Coordinate and maintain client schedules with accuracy and urgency
  • Handle call-outs, schedule changes, and participate in the on-call rotation
  • Ensure both clients and CHHAs feel supported and satisfied
  • Manage EVV daily tasks and resolve clock-in/out issues
  • Document all client and employee concerns promptly and professionally

Caseload Growth & Relationship Building

  • Build and maintain strong relationships with clients, CHHAs, referral partners, and community organizations
  • Process new intakes and service requests quickly and correctly
  • Monitor caseload performance, unfilled hours, payroll accuracy, and billing oversight

Field Staff Support

  • Assist with onboarding new employees
  • Train staff on agency policies, EVV, and expectations

Culture & Retention

  • Help promote retention activities and create a positive experience for field staff
  • Represent NJ Home Health Care as a provider and employer of choice

Requirements

  • Minimum 3 years of verifiable supervisory, scheduling, or coordination experience in home care or healthcare
  • Strong communication and interpersonal skills
  • Demonstrated ability to handle responsibility and grow within a role
  • Ability to multitask, problem-solve, and stay calm under pressure
  • Ability to work positively with clients, families, caregivers, and the office team
