Charity Care Group is seeking a Licensed Practical Nurse (LPN) to provide one-on-one skilled nursing care for a 29-year-old male with a G-Tube attending a Suffolk Day Support Program.
This is a daytime weekday position with consistent hours and the opportunity to provide focused patient care in a supportive environment.

Schedule: Monday – Friday
</li>
<li>
Hours: 9:00 AM – 4:00 PM
</li>
<li>
Status: PRN (3–5 days per week)
</li>
</ul>
Meet &amp; Greets are being held daily at 11:00 AM starting tomorrow.
Responsibilities
<ul>
<li>
Provide skilled nursing care during day support hours
</li>
<li>
Manage G-Tube feedings and medication administration according to the care plan
</li>
<li>
Monitor patient condition and report any changes
</li>
<li>
Maintain accurate nursing documentation
</li>
<li>
Work collaboratively with day support staff and family members
</li>
</ul>
Qualifications
<ul>
<li>
Active LPN license in Virginia
</li>
<li>
Current CPR certification
</li>
<li>
G-Tube experience preferred
</li>
<li>
Experience with individuals with developmental or intellectual disabilities is a plus
</li>
