We’re seeking an RN Case Manager to join our growing team and provide skilled nursing care wherever patients call home. This is an opportunity to practice meaningful, patient-centered care while shaping a new hospice program.
What You’ll Do
- Provide skilled nursing assessments and interventions
- Develop and update individualized care plans with the interdisciplinary team
- Coordinate care with physicians, nurses, and other team members
- Supervise and support Home Health Aides/CNAs
- Educate patients, families, and caregivers on care and symptom management
- Document care and communications accurately and timely
- Provide on-call or PRN support as scheduled
What You’ll Bring
- Current RN license in Oregon
- 1+ year clinical experience (hospice, home health, or LTC preferred)
- Strong communication and teamwork skills
- Valid driver’s license, reliable vehicle, and full coverage auto insurance
- CPR/First Aid certification
