We’re seeking an RN Case Manager to join our growing team and provide skilled nursing care wherever patients call home. This is an opportunity to practice meaningful, patient-centered care while shaping a new hospice program.

What You’ll Do

• Provide skilled nursing assessments and interventions
• Develop and update individualized care plans with the interdisciplinary team
• Coordinate care with physicians, nurses, and other team members
• Supervise and support Home Health Aides/CNAs
• Educate patients, families, and caregivers on care and symptom management
• Document care and communications accurately and timely
• Provide on-call or PRN support as scheduled

What You’ll Bring

• Current RN license in Oregon
• 1+ year clinical experience (hospice, home health, or LTC preferred)
• Strong communication and teamwork skills
• Valid driver’s license, reliable vehicle, and full coverage auto insurance
• CPR/First Aid certification
