Company Overview
Avenidas is a nonprofit organization providing services to older adults and caregivers in the Mid-Peninsula. Avenidas offers a wide variety of health, education, recreation, and social service programs. The Avenidas Rose Kleiner Center provides adult day health care services to frail and dependent older adults, as well as support for family caregivers.
Position Overview
The Avenidas Rose Kleiner Adult Day Health Care Center’s Registered California state-licensed Occupational Therapist is responsible for assessing referred participants who require rehabilitation services, including direct care, to evaluate medical condition, functional capabilities, limitations and restrictions, and potential for rehabilitation. The OT will establish and administer an Individual Care Plan with specific goals determined by each participant’s capacity and tolerance, under the direction of the Health Services Manager. The position may provide clinical supervision and oversight to Rehabilitation Aides.
Essential Job Functions
· Assessment and Person-Centered Care Planning for incoming and active participants.
· Member of the Multidisciplinary Team and assist with carrying out the team’s plan of care.
· Assess functional ability regarding ADL’s (Activities of Daily Living).
· Train participants and staff on proper body mechanics for safety.
· Recommend appropriate adaptive devices to improve independence.
· Conduct in-home evaluations as appropriate for home safety considerations and recommendations.
· Demonstrate safe practices by utilizing proper body mechanics, adhering to universal precautions, and maintaining environmental safety.
· Contribute to the welcoming environment, assisting participants as needed.
· Supervise participants throughout the Center to ensure their safety, proper care, and comfort.
Secondary Job Functions
· Attend all staff meetings, training, and in-services.
· Perform other related duties as assigned.
Qualifications
Education and Experience
· Must have graduated from an accredited school.
· Current California state license as a Registered Occupational Therapist.
Knowledge, Abilities, and Skills
· Ability to develop a plan of care for each patient.
· Skilled in providing occupational therapy services/interventions.
· Ability to assess participant needs and to plan, evaluate, and modify care to meet the goals of OT interventions.
· Interest, ability, and experience working with seniors in a multicultural environment.
· Must have a valid California Driver’s license, clean driving record, appropriate insurance, and reliable vehicle.
Physical Demands and Work Environment
· Ability to lift, transfer, push/pull, maneuver, and reposition 35 lbs.
· Ability to reach, bend, and walk.
· Frequently stand/walk, sit, use a computer, use a telephone, and grasp lightly/fine manipulation.
· Occasionally reach/work above shoulders, twist/bend/stoop/squat, grasp forcefully, write by hand, sort/file paperwork.
· Regularly required to push wheelchairs.
· TB and health clearance required.
Hours and Compensation
This is a fully benefited, part-time (20 hours per week), non-exempt position. The role requires on-site presence from 8:30 a.m. to 2:00 p.m., with flexibility regarding specific workdays. The current salary range is $56.84 – $59.52.
