Job Summary
The Care Coordinator / Medical Assistant provides both administrative and clinical support to individuals and families seeking care at Plymouth Psych Group. This role is responsible for front-desk coordination, patient intake, scheduling, and communication, while also supporting clinical staff with basic medical assistant duties such as rooming patients, documenting vitals, and assisting providers to ensure efficient, high-quality patient care.
This position serves as a liaison between patients, providers, clinical staff, and the back office while maintaining strict confidentiality and professionalism.
Duties & Responsibilities
Care Coordination & Administrative Duties
- Greets all patients and verifies demographic and insurance information for accuracy.
- Assists patients and family members with completion of pre-registration and intake forms.
- Schedules, reschedules, and coordinates appointments, including last-minute and urgent scheduling needs.
- Reviews no-call/no-show accounts and contacts patients to reschedule appointments.
- Answers phones, screens callers, relays messages, and routes calls to appropriate departments.
- Checks voicemail every 30 minutes to ensure timely response to patient messages.
- Confirms callbacks, reschedules appointments, and communicates no-show/late cancellation policies.
- Maintains, files, and updates patient charts, orders, demographics, and other documentation; shreds outdated information per policy.
- Informs patients of costs of care and collects copays, coinsurance, and deductibles.
- Posts payments according to protocol; prepares and balances daily financial registers and submits documentation to billing.
- Maintains physician and provider schedules within the Practice Management system.
- Assists with filing and other administrative support needs as assigned.
Medical Assistant Duties
- Rooms patients and prepares them for appointments in a timely and professional manner.
- Obtains and documents vital signs, height, weight, and other relevant clinical information as appropriate.
- Assists providers with clinical workflows, including pre-visit preparation and post-visit follow-up tasks.
- Updates medication lists, allergies, and patient histories in the electronic health record (EHR).
- Prepares exam rooms and ensures rooms are stocked, clean, and organized.
- Assists with coordination of lab orders, referrals, prior authorizations, and documentation as directed.
- Provides patients with instructions, educational materials, and follow-up guidance per provider direction.
- Communicates patient concerns or clinical observations to providers promptly and accurately.
- Supports telehealth visits by assisting with patient check-in, documentation, and troubleshooting as needed.

- Basic computer proficiency, including Microsoft Word and Excel; experience with EHR systems preferred.
- Medical Assistant experience or training preferred (certification a plus but not required, if applicable).
