Position Summary: The Non-Medical Case Manager (NMCM) is responsible for providing a range of client-centered activities focused on improving access to and retention in needed core medical and support services for individuals living with HIV/AIDS. This role involves coordination, guidance, and assistance in accessing medical, social, community, legal, financial, employment, vocational, and other needed services.
Key Responsibilities:
- Client Assessment and Planning:

- Conduct comprehensive assessments to identify clients’ medical, psychological, and social needs.Develop personalized care plans to address identified needs and goals.
Service Coordination:
- Facilitate access to medical, social, community, legal, financial, employment, and vocational services.

- Assist clients in obtaining access to public and private programs such as Medicaid, Children’s Health Insurance Program, Medicare Part D, State Pharmacy Assistance Programs, and other supportive services.
Client Support and Advocacy:
- Provide ongoing support and advocacy to help clients navigate healthcare and social service systems.

- Offer emotional and psychological support, including counseling to help clients cope with stigma and mental health issues.
Resource Connection:
- Connect clients with resources for transportation, housing, financial support, and food services.

- Ensure clients are aware of and can access all available benefits and entitlements.
Retention and Adherence:
- Support clients in adhering to antiretroviral therapy (ART) through education and reminders.

- Monitor and promote retention in care, aiming for sustained viral suppression and compliance with medical care.
Crisis Intervention:
- Provide immediate support during emergencies, ensuring clients receive timely and appropriate assistance.
Documentation and Reporting:
- Maintain accurate and up-to-date client records in the electronic health record (EHR) system.
- Document all case management encounters, including face-to-face, telehealth, phone contact, and other forms of communication.
Qualifications:
- Bachelor’s degree in social work, psychology, public health, or a related field.
- Experience in case management, preferably in a healthcare or social services setting.
- Knowledge of HIV/AIDS and related health and social issues.
- Strong organizational and communication skills.
- Ability to work collaboratively with clients, healthcare providers, and community organizations.
- Proficiency in using electronic health record (EHR) systems.
Preferred Qualifications:
- Bilingual (English/Spanish or English/Creole) to better serve the diverse population of Broward County.
- Certification in case management or related field.
Working Conditions:
- Office-based with regular travel to meet clients and attend community meetings.
- Flexible hours, including some evenings and weekends, to accommodate client needs.
