Patient Care Coordinator II

Core Responsibilities
You will manage a panel of patients and be responsible for ensuring they receive timely, high-quality care.
- Own the full patient lifecycle: onboarding, matching, engagement, and follow-through
- Match patients with doulas based on preferences, geography, and availability
- Proactively manage patient communication and engagement
- Coordinate scheduling and ensure visits are completed
- Resolve issues including cancellations, rematching, and care gaps
- Track patient progress and ensure continuity of care using our AI powered care tools
- Identify risks early and escalate when needed
- Maintain accurate records and workflows in internal systems
Preferred Experience
- You have experience managing patients, clients, or cases end-to-end
- You are highly organized and can manage multiple patients simultaneously
- You communicate with empathy, clarity, and confidence
- You take ownership—nothing falls through the cracks
- You are resourceful and solution-oriented
- You are comfortable in a fast-moving, startup environment
Qualifications
- 2–4+ years in healthcare coordination, care navigation, or patient support
- Experience with Medicaid, community health, or maternal health
- Experience working with doulas, case management, or perinatal care
- Familiarity with CRM or care coordination tools
- Multilingual is an advantage
Compensation
- $60,000 – $75,000 base salary
- Performance-based incentives
