NP Requirements Summary
Facility Address: 300 Werner St, Hot Springs, Arkansas, 71913-6406
Education/Training:
• Graduate of an accredited Nurse Practitioner program
Clinical Experience:
• Prior EM experience required; Documentation of Clinical Experience (i.e. logs)
License:
• Active unrestricted AR NP license, DEA license w/ AR address and prescriptive authority
• Active Arkansas licensure & DEA/CDS required
Board Certification:
• AANP/ANCC; Board Certification required
Merit Badge:
• ACLS, PALS & ATLS
Immunizations:
• COVID Vaccination
• TB Screening (within 12 months)
• Flu Vaccination (within 12 months)
• Hep B Vaccine (3 Doses) or Titers
• MMR Vaccine (2 Doses) or Titers; Varicella Vaccine (2 Doses) or Titers
• Tdap (within the last 10 years)
Case Log:
• Two (2) years of case logs
CME/CEU:
• Two (2) years of CMEs
Specific Facility Procedures:
• Must have a Supervising Physician assigned; Broad EM skills set appropriate for a critical access facility
• Malpractice Coverage: $1M/$3M
• Additional Details:
o Current Resume;
o Driver's License; color photo;
o TB Test/Flu in last 12 months;
