Schedule: On-call and available for visits, 5:00 PM - 8:30 AM, 7 on / 7 off.

Your Day-to-Day at Custom Hospice:
- Deliver direct patient care in accordance with the State Nurse Practice Act.
- Implement care plans developed by the registered nurse.
- Document care provided accurately and promptly, following the established care plan.
- Educate patients and families on self-care techniques, diagnoses, and treatment plans.
- Collaborate with interdisciplinary teams, identifying needs for additional services (e.g., OT, PT, MSW, ST, Dietician).
- Utilize and maintain equipment and supplies efficiently while adhering to aseptic techniques.
- Participate in staff meetings, training sessions, QAPI, case conferences, and Emergency Preparedness Training.
- Foster your personal and professional growth through ongoing learning opportunities.
- Perform additional duties as assigned by the registered nurse.
What We’re Looking For:
- Graduate of an accredited practical nursing or vocational nursing program.
- At least 1 year of nursing experience (home health, hospice, or medical/surgical preferred).
- Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in the state.
- CPR Certification: Must be valid and current.
- Strong communication and organizational skills.
- Licensed driver with a reliable, insured automobile that meets organizational requirements.
