Job Description

Job DescriptionWe’re hiring a Case Manager in North Hollywood, CA. You will be a part of a dynamic team to provide transformative care and support to individuals in interim housing. This role offers you the opportunity to shape lives, foster empowerment, and contribute to innovative solutions in the social services field.

What You’ll Do:

  • Build strong relationships with mental health providers, hospitals, and the Department of Mental Health

  • Lead discharge planning and care coordination

  • Apply Harm Reduction, Housing First, and Trauma-Informed Care principles

  • Collaborate with medical teams to triage participants to appropriate care

  • Conduct risk assessments and coordinate with PMRT when needed

  • Provide crisis intervention, medication support, and conflict resolution

  • Supervise MSW interns and support onboarding of new hires

  • Maintain accurate documentation in HMIS/CHAMP

What We’re Looking For:

  • Master’s degree required in MSW, MFT or related field and have an LPCC registration #

  • A minimum of 5 years of experience in case management, crisis intervention, and group facilitation

  • Bilingual in Spanish highly preferred

  • Strong understanding of mental health systems and community resources specifically to help those who are facing housing instability

  • Ability to work collaboratively in an interim housing setting (no field work)

Pay Details: $30.00 to $34.00 per hour
