Job Summary

The Home Health Care Coordinator plays a key role in managing patient care schedules, supporting caregivers, and ensuring smooth day-to-day operations. This position requires strong communication skills, attention to detail, and the ability to multitask in a fast-paced environment.

Key Responsibilities

• Coordinate and manage caregiver schedules for home care services
• Communicate with patients, families, and healthcare professionals
• Maintain accurate patient records and documentation
• Assist with onboarding and supporting caregivers
• Handle incoming calls and respond to service inquiries
• Ensure compliance with company policies and healthcare regulations

Qualifications

• High school diploma or equivalent required
• Previous experience in home healthcare or administrative support preferred
• Strong communication and organizational skills
• Ability to work independently and as part of a team
• Basic computer proficiency (email, scheduling systems, data entry)
