Position Highlights
*Potential eligibility for a $30,000 sign-on bonus!
HOURS:
8:00 AM - 4:30 PM (flexible); every fifth weekend and holiday rotation; 72 hours over two weeks

Overview
This position is responsible for organizing and coordinating information from various sources to create and implement a personalized, age-appropriate, collaborative care plan.
Key Responsibilities
- Manage staffing to fulfill the needs of residents, delegating tasks appropriately and ensuring follow-up for completion.
- Conduct timely assessments and prepare residents and families for transitions to the next level of care.
- Effectively communicate relevant resident information to the healthcare team, residents, and families in a clear and timely manner.
- Document Medicare assessments and interventions, ensuring diagnostic and lab reports are accurately filed in medical records.
- Transcribe physician orders and accompany providers during rounds to note orders or data entries that require completion.
- Accurately record clinical data received from nursing and medical staff.
- Maintain organizational systems, including monthly ADL, behavior sheets, and BM boards.
- Prepare daily Medicare sheets and lab slips, as well as charts for physician rounds.
- Schedule annual medical reassessments, dental evaluations, and pain assessments.
- Propose positive changes and enhancements within the department and facility to elevate the quality of care and service, fostering a supportive environment for residents and staff.

Required Education, License/Certification, or Work Experience:
- Licensed Practical Nurse (LPN) - Must possess an active license from the Board of Nursing in the state of practice upon hire.
Preferred Education, License/Certification, or Work Experience:
- 1-3 years of relevant experience.
