About the Role
Provide comprehensive primary care services to patients in their homes or senior living communities.
Responsibilities
- Serve as a Bloom ambassador in the San Antonio market, contributing to market development and local healthcare landscape analysis
- Strategically grow and manage your patient panel, delivering approximately 10 in-home visits per day
- Build strong relationships across the care continuum, including assisted living facilities, hospitals, clinics, and community organizations
- Collaborate with interdisciplinary teams—specialists, pharmacists, and social workers—to coordinate care and improve patient outcomes
- Deliver condition-specific specialty visits as needed
- Utilize population health data to drive clinical decision-making and quality outcomes
- Participate in ongoing training, education, and development opportunities
- Travel occasionally to Denver for initial and ongoing training
- Commute to assigned assisted living communities to conduct home-based visits
Qualifications
- Active, unrestricted Texas medical license
- Commitment to high-quality, patient-centered primary care
- Interest in innovative, value-driven, in-home care delivery
- Ability to collaborate effectively with interdisciplinary teams
Required Skills
Our physicians are supported by a robust clinical and administrative care team including MDs, Nurse Practitioners, triage staff, marketing ambassadors, IT, billing, and scheduling teams. Pay range and compensation package
Salary is competitive and commensurate with experience. The typical range for this role in Texas is $225,000–$260,000 annually, with additional benefits. Actual compensation may vary based on qualifications, experience, and location.
