At Ollie Hinkle Heart Foundation, we support children and adults living with congenital and childhood-onset heart conditions—and the families who love them. Our work sits at the intersection of medical complexity, mental health, and trauma-informed care, and we’re growing.
We’re looking for a licensed or license-eligible case manager who believes that healing happens when people feel seen, supported, and not alone 💙
About the Role
This is a 20–30-hour-per-week hybrid position, preferably for asocial worker with trauma-informed care experience who wants to make a real impact without burning out.
You’ll work directly with heart patients and families navigating medical trauma, mental health challenges, and complex systems—while being part of a collaborative, values-driven nonprofit team.
- Because this role includes in-office collaboration, candidates must live in the St. Louis area.
What You’ll Be Doing
- Provide trauma-informed care coordination and case management for individuals and families affected by congenital and childhood-onset heart conditions
- Support clients navigating medical systems, mental health resources, and community services
- Collaborate with therapists, care managers, and program leaders to deliver whole-person care
- Build trusting relationships with families experiencing high stress, grief, or medical trauma
- Document care and progress thoughtfully (HIPAA-compliant, of course)
- Participate in team meetings, supervision, and occasional community events
What We’re Looking For
- A Social Worker or equivalent; licensure or license-eligible preferred
- Experience with trauma-informed care (medical trauma experience is a big plus)
- Comfort working with families under stress and navigating complex care systems
- Strong communication, empathy, and organizational skills
- Ability to work both independently and collaboratively
- Must be based in St. Louis, MO
